Personal health statement for driving license

    A personal health statement is required in a number of cases when applying for a driving license.

    You will need to supply a personal health statement if:

    • your driving license expires on or after your 75th birthday.
    • you need a driver's license for a lorry or a bus;
    • you have a medical limitation.

    The application for the personal health statement can be done in three ways:

  • How to

    You can apply for the personal health statement at the municipality or at the CBR. After completion of the first part of the form you need to make an appointment with a doctor. See for information about which doctor you can approach (only in Dutch). The doctor completes the form further.

    When everything is complete, send the personal health statement to the CBR. You will receive a letter from the CBR stating whether or not your application has been approved. In some cases the CBR may require an additional medical examination or driving test, so that a better estimate of your situation can be made.

    Your bank card (payment by cash is only possible at the customer center in Veghel).

    Note: this is the fee for the personal health statement only. Any extra costs (ie medical examinations) are not included.

  • How long does it take?

    When you order online the personal health statement will be sent within 5 working days.
    When you apply at the customer service desk you receive the personal health statement immediately.
